Although interpreter standards are in place today, monitoring practices vary widely among health systems and language service providers. Interpreter quality evaluations are typically manual with humans reviewing a sampling of sessions to ensure consistent quality and user experiences across the board. Relying on sampling can be especially problematic for large healthcare organizations averaging more than 100,000 minutes or more of video and audio remote interpreting each month. At high volume, humans can only evaluate a small fraction of interactions to identify and reduce language barriers.

The high cost of no or poor interpreting 

With more than 68 million people in the United States speaking a language other than English, accurate medical interpretation isn't just a compliance requirement—it's imperative for patient safety, experience, and clinical outcomes. In one study examining readmission rates and the length of hospital stays for limited English proficient (LEP) patients, researchers found that approximately 24% of those who did not have access to an interpreter during admission and discharge were readmitted within 30 days. Additionally, patients who used an interpreter had a significantly shorter stay than LEP patients without an interpreter (2.57 days vs. 5.06 days).

A poor interpreting experience also puts patients at risk, with the National Institutes of Health (NIH) blaming incorrect diagnoses, missed symptoms, wrong medication dosages, inappropriate treatment plans, delayed care, non-compliance with treatment instructions, and even patient harm, all on miscommunication due to language barriers when a qualified interpreter is not used or when the interpretation is inaccurate.

Without adequate language support, LEP patients are less likely to understand their diagnosis, treatment plans, or follow-up care and feel confident about the care they receive. They are also more likely to have negative experiences with their care providers.

According to Jennifer Winters, assistant vice president of Value Based Care at Affinia Healthcare, high-quality interpretation from the start of care is fundamental. "Recognizing the patient's cultural and linguistic needs can create a greater level of understanding, lead to better outcomes, and reduce repeat visits, as it allows healthcare providers to see a more complete picture of the patient's health," she said.
